Targeted Case Management (TCM)

Targeted Case Management(TCM) focuses on each individual’s strengths and abilities. TCM assists individuals by helping them identify those strengths, linking them to resources in the community, and helping them identify and overcome any barrier that may hinder them from succeeding in a goal.


Clients who are eligible to receive TCM must meet medical criteria and have Kentucky Medicaid or Kentucky MCO as their insurance.

Community Support Associates are essential in assisting individuals facing mental health challenges or requiring social support, enabling them to live more independently and engage more actively with their communities.


Their responsibilities encompass: 

offering direct support and coaching in daily living skills, social interactions, and vocational training. They collaborate closely with clients to recognize their strengths, interests, and aspirations, crafting personalized plans that encourage self-sufficiency and elevate quality of life. 


Community Support Associates also help connect clients to community resources and services, such as educational programs, recreational activities, and healthcare services, ensuring a comprehensive approach to support. By advocating for their clients’ needs and rights, they work to dismantle barriers to inclusion and access to services. Their commitment empowers individuals and cultivates a more inclusive and supportive community environment.
